Reizdarm bei Kindern (funktionelle Bauchschmerz-Syndrome) ist nach Rome-IV-Pädiatrie eine etablierte Diagnose ab dem 4. Lebensjahr: die Prävalenz in deutschen Studien liegt bei 5–15 % im Schul- und Jugendalter. Die Diagnose gehört in kinderärztliche Hände; selbst-diagnostische Ansätze sind nicht altersangemessen.

Reizdarm bei Senior:innen ist eine klinisch knifflige Kategorie: die Prävalenz der klassischen Rome-IV-IBS-Diagnose sinkt im höheren Alter leicht, gleichzeitig steigt das Risiko organischer Differentialdiagnosen steil an. Neu auftretende IBS-ähnliche Symptome ab 50 gehören deshalb IMMER zur vollen gastroenterologischen Abklärung.
